Return to school Reminders Annual Health Forms: The district is moving to updating this information electronically. You should have received an email for Portal Account Activation, if you haven’t already. Please follow the steps in the email. This will allow you to update both demographic and health information and give permission for over-the-counter medication to be given at school. Please let me know if you have any questions. Masks: Optional Daily Screening Tool: Here is the daily Pre-Screening Tool. Attend school when all answers are NO and your child is feeling well with no other symptoms of illness. Call or see your school nurse if you have questions. Illness & close contact: If your student is ill: If your student becomes ill or has a positive COVID-19 test, please email me so that we can determine a back-to-school date for your student. If your student is a close contact of a known positive COVID-19 case: If your student is identified as a close contact please email me. These will both require a quarantine period that would impact your student's return to school. Free Thermometers - If your family would like one to be sent home please just let me know and we will be sure to get you one. You can email me at dugasan@spsdme.org. Peanut/Tree Nut Free Classrooms: All classrooms are peanut/tree nut free. Please do not send snacks that will be eaten in class with peanuts or tree nuts. There are Nut Free tables available at lunch. We appreciate your help with this! Water Bottles: Please remember to send your student in with a reusable water bottle. District Nursing and Health Services - Health Alerts, School Health Management Guidelines, Health Articles, Forms and Links are all available at this site. Medications at School We are able to keep student medication in the main office, when necessary. Parents/Guardians need to bring in the medication and complete a form in order for the school nurse to review. Students who keep medication in the office at school will be bringing them home on June 12th unless parental request to pick the medication up is received by office staff or the school nurse prior to that date. Parents of students with medication that MUST be picked up will be notified directly by the nurse. Thank you.
